Statement giving the staff profile. The details therein should be filled by every faculty/staff members.

Sanctioned programmes along with annual intake in the institution.

6.

Name of faculty and Staff in full as mentioned in school certificate along with their qualifications, scale of pay and photograph.

7.

Name of faculty members who joined or left during the last quarter.

8.

Students admitted during the current session along with qualification, percentage of marks in the qualifying examination and in the entrance test, if any, date of admission, etc.

9.

Fee charged from students.

10.

Available infrastructural facilities.

11.

Facilities added during the last quarter.

12.

Number of books in the library, journals subscribed to and additions, if any, in the last quarter.

Copies of the FDRS (Endowment Fund of Rs. Five Lakhs and Reserve Fund of Rs. Seven Lakhs) for five years converted into Joint Account with the Regional Director
